A CASE OF FEVER WITH VOMITINGS SINCE 5 days.



 October 8th, 2021

AMC BED ADMISSION. 

A 38 year old female patient came to the casuality with complaints of FEVER since 5 days. 

                      VOMITINGS since 5 days.

Patient was apparently asymptomatic 5 days back when she developed FEVER with chills and is associated with cold, mild productive cough(white sputum) and headache. The fever was more in the evening and subsides in the morning and subsides for a few hours after taking medication. The fever was also associated with non-projectile VOMITINGS of around 5 per day. Patient also complaints of frequent BELCHING round the day.  Generalised weakness - present. 

The patient’s mother died 14 days ago, that lead to her loss of appetite. 

No c/o chest pain, sob, palpitations. 

No c/o involuntary movements. 

PAST HISTORY-

Not a k/c/o HTN, TB, DM, EPILEPSY, ASTHMA. 

Non- smoker 

Non- alcoholic

VITALS ON ADMISSION- 

BP- 70/50 mm hg 

PR- 100bpm

RR- 22 cpm

Temp- 

Spo2-97% on RA

GRBS- 


ON EXAMINATION- 


CVS- S1S2 heard

RS- BAE +, No crepts 

CNS- NAD 

P/A - Soft, non-tender

COURSE IN THE HOSPITAL- 

Day 1- 

IVF - NS, RL @ 100ml/hr

INJ. Zofer 4mg IV/TID

INJ.PAN 40mg IV/OD

INJ. Optineuron 1amp in 100NS/ IV/ OD

BP MONITORING 2nd hrly

CKD 2° AKI due to SEPSIS

 The patient came to the opd with the chief complaints of -  LOW BACK ACHE SINCE 1week  LOSS OF APPETITE SINCE 1week  FACIAL PUFFINESS SINCE 1week  SHORTNESS OF BREATH (grade-III) HOPI-  Before 20yrs- an accident leading to splenectomy. 2019- The patient was apparently asymptomatic 2yrs back when he had an injury on the Dorsum of right foot leading to ulceration.  He went to the local hospital for the same and was found to be anemic (Hb-5.3). He was then referred to Khammam.  In Khammam- they found out the was some derangement in the kidney from USG, RFT. He was told to have a kidney infection due to sepsis from the ulcer.  He was discharged in a couple of days.  October,2021-  He was alright until 2months back, when he developed- FEVER, COLD, PEDAL EDEMA(PITTING TYPE), WEAKNESS.
